Please send a screenshot of your King Soopers Community Rewards quarterly statement to GroceryReimb@FairmountPTA.org.  You can access your quarterly statement through the King Soopers app (My Account > Community Rewards) or at www.KingSoopers.com.

Please include the name of the owner of the Sooper Card, your name, and your youngest child’s name in your email.  Once your screenshot is received, we will record it and add it to your PTA account.
